Departmental policy concerning the creation of reports is as follows:-

Summary:- Each report will be started with a brief summary of findings of the report

e.g.  This agent was requested and required by the department to investigate ……………………
        This agent has looked at all information  concerning this investigation and has found (or not found) 
Proof that …………………………… was happening. Report follows:-
 

Introduction:- Each report will include an introduction indicating what the topic deals with and how it was to be investigated and by whom.

e.g. This  report on …………………………………… by agent ………………….looks into the subject very carefully. For many years people have been mentioning/suggesting/reporting the existence of ……..
Even as early as ……………………… information concerning this has been found etc.,

In this section agents may wish to include references to other information or books on the subject.

Main Investigation:- Each report will include a main section where the investigative evidence and items that the agent are included. This part of the report should include any apparatus, plans, strategies or research items that were considered.

e.g. To conduct this investigation agent ………….. decided to enlist the help of ………
       This investigation included the use of ……………………….
       Some of the evidence we/I considered included…………….
       In the book by …………….. it was indicated/mentioned that …………
       An interview with ……………. gave the following information:- 

Conclusions:- Each report will include a conclusion  indicating what the agent thought was the truth based on the evidence and investigative  methods used. Agents can, at this point, suggest other possible information they may have wished to get and suggestions for methods used in future investigations.

e.g. In conclusion it is this agent’s opinion that …………………does/does not exist/occur and based on the findings above. I/We have given this matter serious thought and it seems apparent that …etc.,
We/I further recommend that future investigations should consider………etc.,
My personal opinion of this is …………etc.,
 

Bibliography:- Each report will include a bibliography  of sources and materials used in the investigation.

ref:- UFO Study……..J. Randles

ref:- Interview with XXXXXX concerning their encounter
ref:- Observation based on the night of XXXX by eye-witnesses
 

ALL REPORTS MUST BE SIGNED AND SUBMITTED TO THE OFFICE WITHIN THE CORRECT TIME FRAME
